CLINIC CONCIERGE MEMBERSHIP

$1500/YEAR or $250/month for 6 months

Concierge membership grants you the ability to have unlimited 24/7 access to us. This plan includes a one-time set up fee or in monthly installments that is good for one year. The cost of our plan is affordable and probably less than you are spending out of pocket yearly even utilizing your insurance plan.

VIP CONCIERGE MEMBERSHIP

$3,000 per year OR $250 per month for 12 months

• Yearly wellness exam with follow up post lab discussion

• Annual Lab Panel:  CBC, CMP, Lipids, TSH, Free T3, T4, Vitamin D, B12, FSH, LH, Total Testosterone, Estradiol, PSA (men) 

• Can be used for any service within our facility: This includes our family medical practice, hormone therapy, medical spa services and aesthetic procedures.

• Same day if not next day office/telemedicine appointments for illness.

• Priority scheduling for non-emergent visits

• 10% Off products, office procedures and med spa services

• Unlimited text/email/phone communication with staff

• Home visits available during times where one is unable to leave the home and telemedicine is not an acceptable means of assessment

AFTER HOURS METHOD OF CONTACTING US FOR CONCIERGE MEMBERS

Our clinic days are Monday- Friday with lab and IV therapy available during those hours. Special circumstances such as late visits or home visits are coordinated by our patient liaisons and the providers. Yes, we will sleep but if an emergency develops after clinic hours you will contact our staff through a private cell number. A provider or staff member will get to you as soon as possible. If you need to report to the Emergency Department, we will collaborate with ER staff on your problem, history, and can electronically send them any needed documentation to improve their care to you. If needed, outpatient visits and procedures can be done by the provider and will be organized by our staff with the selected facility prior to treatment.

For our concierge members, this 24/7 access is unlimited. Unlimited text, email, telemedicine, same day if not next day appointments.

PRESCRIPTION POLICY

• We will send your prescription to your pharmacy of choice electronically or by phone.

• No controlled substances will be electronically faxed or called unless you have been evaluated by one of our providers and documentation is available.

• Pain medication refills are handled following our pain management contract. You will not be able to use another provider. You will be obligated to use the same pharmacy at all times. You will not be refilled early prior to the 30 days given. You may not go to an emergency department and request more or a change in meds. Random drug screening must be accepted by the patient at any time. This is a very black and white subject and a system that doesn’t waver with many checks and balances. Any violation of the above can lead to disengagement of a patient from our practice. Any arguing or mistreating of our staff or pharmacy staff can also give our office grounds for immediate disengagement of a patient.

CANCELLATIONS

All appointments require a $50 non-refundable deposit that will be charged at the time of booking for facials, laser treatments, JetPeel treatments and any appointment with our nurse practitioners.  This amount will be applied to the final bill for your appointment. 

If you must cancel, for any reason, we request that you CALL US AT LEAST 24 HOURS in advance.

For appointments canceled at least 24 hours in advance, the deposit for your missed appointment will be applied to your next appointment.

For appointments canceled less than 24 hours in advance (including no-shows), your deposit will be retained as a cancellation fee, and you will be required to pay a new deposit when booking your next appointment.
